- Full-time day shift at GMF Fusion in St-Hyacinthe;
- Full-time day shift at GMF Solidarity Health Cooperative in the MRC of Acton.

Make our community better by
- Assessing the patient and family's ability to perform self-care and manage their health;
- Informing the person and family about professional resources that can help them;
- Providing follow-up care for a clientèle aged 0-100 years (systematic and joint follow-up);
- Promoting health, preventing disease, evaluating, following up, and coordinating care and services;
- Initiating and performing diagnostic exams and tests according to individual and collective orders;
- Directing patients to relevant services and ensuring communication between these services and the patient (SAD, 24/7, Info-Santé, other professionals, community organizations, second line).
- Essential qualifications (Health and Social Services requirements):
- Hold a bachelor's degree in nursing sciences;
- Licence from the Order of Nurses and Nurses of Québec (OIIQ).
- Note that as a flying team, you must be able to travel throughout the Richelieu-Yamaska territory: Beloeil, Saint-Hyacinthe, St-Bruno-de-Montarville, or Saint-Hyacinthe and Acton Vale.
