In collaboration with the care director and the different interdisciplinary teams, the nurse evaluates the physical and mental condition of the residents, organizes their care plans and ensures medical follow-ups with external teams. She also coordinates care with the care teams, both at Humanitæ and at home.

Tasks

• Evaluate the physical and mental health of residents and administer the necessary screening and measurement tests;
• Perform the medical rounds with the doctor;
• Evaluate the resident's functional autonomy using the SMAF;
• Ensure and coordinate the internal and external follow-ups required by the resident's health condition;
• Develop, enter into the SYMO software and apply interdisciplinary intervention plans;
• Ensure that care complies with quality standards, practice standards and certification rules for residences;
• Participate in the accompaniment of the relatives facing the realities of people with memory disorders upon arrival or return;
• Coordinate continuous training and supervise care teams;
• Ensure that the approach and mission of Humanitæ are applied by the different teams;
• Perform other related tasks.
