Summary of Duties
- Under the authority of the head of the social services department, the incumbent:
- Conducts psychosocial evaluations of patients and/or families for the purpose of assessing the need for service, the nature of the problem, and what has led to the formulation of appropriate recommendations and treatment plans in an acute care setting;
- Develops a collaborative intervention plan with the patient and their family if necessary, in a collaborative approach.
- Conducts psychosocial evaluations for the opening of protection regimes, as needed.
- Plans with patients and/or family members as well as with the multidisciplinary team, the organization of the leave. Ensures a link between the patient who has received their leave and the appropriate community resources as needed.
- Collaborates closely with the multidisciplinary team to ensure complete care for the patient and to sensitize multidisciplinary colleagues to the psychosocial aspects of illnesses and disabilities. Offers consultations to the team on psychosocial issues.
- Organizes and facilitates support groups (if applicable) according to identified needs.
- Acts as an advocate for the needs and rights of patients within and outside the hospital and ensures the maintenance of health, safety, and dignity of their person during hospitalization and particularly during the planning of leave;
- Participates in the evaluation of social service needs in the assigned area;
- Identifies gaps in the network of resources for services and patient populations assigned;
- At the Master's level in social work, collaborates with educational institutions to provide field teaching for first-year social work/master's in social work and social counseling programs;
- At the Bachelor's level in social work, collaborates with educational institutions to provide field teaching for students in bachelor's in social work and social counseling programs;
- Works in collaboration with the director of the social work department to develop and implement new social work service delivery systems to meet the ever-changing needs of the patient population;
- Participates in research activities related to the psychosocial aspects of illnesses and disabilities and applicable social service delivery systems to the service's activities;
- Participates in social work department meetings;
- Participates in hospital and service administrative and organizational committees to plan and provide all hospital services, as required;
- Commits to working on-call on a weekend rotation basis, the frequency of which may vary;
- Assumes full responsibility for coverage for other colleagues during vacations, sick days, and absences, in accordance with the service's policy and the needs of patients;
- Provides social work services on a rotating basis for the "Intake Service Module" of the social services department;
- Manages workload and responds to administrative requirements concerning registration, statistics, and any other need for workflow document reports and facilitates communication.
