The Champlain Group Inc. is a grouping of 13 private, contracted Long-Term Care Facilities (LTCFs), located in several regions of Quebec, whose mission is to offer quality, deeply human, continuous care and services in a secure living environment to people with complex needs throughout their life journey in housing.

Our team and our residents are currently looking for a DIETITIAN-NUTRITIONIST 2 days per week embodying the values of Kindness, Excellence, and Responsibility.

Participate in the healthy and balanced nutrition of our residents by:

  • Assessing the nutritional status and specific needs of our residents;
  • Developing and implementing clear and concise procedures for the nutrition service and all associated partners;
  • Helping to prevent and treat the consequences of inadequate nutrition for our residents;
  • Specific Functions:

Develops intervention plans using a scientific approach; designs, organizes, executes, and evaluates intervention plans within their professional competence addressing complex or varied nutritional issues.

Evaluation

  • Assesses and reassesses nutritional status;
  • Analyzes, interprets, and identifies nutritional issues;
  • Assesses the functional capacity to eat;
  • Assesses an individual's eating behavior;
  • Develops and adjusts the nutritional intervention plan to the user's living conditions.
  • Treatments
  • Writes and applies the nutritional intervention plan in consultation with the interdisciplinary intervention plan of the user;
  • Links with relevant organizations for users requiring ultra-specialized food products, equipment, and materials;
  • Provides counseling, including individual or group teaching;
  • Evaluates the user's progress, considers treatment reactions, and adjusts interventions accordingly;
  • Writes progress notes and maintains files according to professional standards and facility policies;
  • Acts as a consultant to the family, user, interdisciplinary team members, and colleagues;
  • Communicates clinical information to interdisciplinary team members and health and social services partners to ensure user follow-up in the service continuum;
  • Ensures necessary follow-ups with the user and ensures the implementation of the treatment plan in accordance with best practices;
  • Participates in interdisciplinary team meetings and meetings related to their function; and updates the interdisciplinary intervention plan in collaboration with the interdisciplinary team.
  • Other Tasks
  • Keeps up-to-date with knowledge related to their field of activity;
  • Completes periodic professional activity statistics according to discipline policies;
  • Regularly checks renewable nutrition materials and informs their supervisor of materials to be ordered;
  • Seeks assistance from staff and determines some of their activities in connection with their area of expertise;
  • Performs any other tasks as requested by their immediate supervisor.

  • Hold a bachelor's degree in dietetics or nutrition;
  • Be a member in good standing of the Professional Order of Dietitians of Quebec;
  • French spoken and written;
  • Professional autonomy;

  • Good organizational skills;
  • Open-mindedness and adaptability.
