Social Worker - Family Planning Clinic

Mandate

Person who evaluates social functioning, determines an intervention plan and implements it. She supports and restores the social functioning of the person in reciprocity with their environment with the aim of promoting the optimal development of the human being in interaction with their environment. She carries out activities of design, orientation, consultation, updating, analysis and evaluation in one or more social programs.

Ethical and reflective practice

• Identify laws, regulations, social measures and policies as well as organizational contexts and consider their impact on practice and potential or proven ethical issues;
• Interpret and apply with discernment the obligations inscribed in the Code of Ethics and the Code of Ethics of the CIUSSS Centre-Sud-de-l'île-de-Montréal;
• Promote and strengthen the power of action of individuals and communities in the exercise of their rights, the fulfillment of their social roles, their interpersonal relationships and their civic participation in relation to social development;
• Adjust interventions to their mandate and responsibilities of protection, supervision, control or assistance according to the vulnerability of the user;
• Base practice on the defense of human rights and the promotion of social justice principles.
• Social intervention process with individuals, families, groups and communities
• Design and carry out activities of information, detection, screening, prevention, appreciation, evaluation, orientation and intervention, including suicide prevention, for the benefit of an individual, a couple, a family and a community;
• Evaluate social functioning in a systemic perspective by integrating a critical analysis of the social aspects that influence the situations and problems experienced by an individual, a couple, a family and a community;
• Evaluate a person with a mental or neuropsychological disorder attested by a diagnosis or an assessment carried out by a qualified professional (activity reserved #4, Law 21);
• Formulate a professional opinion supported by the evaluation of the situation which specifies the priority interventions and the chosen orientation;
• Communicate the conclusions of their clinical judgment while ensuring that they do not harm vulnerable clienteles;
• Develop an intervention plan or concerted strategy with the full participation of the user, determine the appropriate intervention method, carry out the intervention and evaluate it;
• Carry out the follow-up with the user in order to consolidate the achievements, revise their PI or close a service episode;
• Evaluate and carry out any psychosocial intervention required by the situation of individuals, families or groups in crisis or emergency;
• Evaluate the person in crisis according to the modalities of application of the Law on the protection of persons whose mental state presents a danger to themselves or to others;
• Develop the care of the environment for collective solutions to mental health problems and involve local actors in mental health promotion activities;
• Ensure a compliant file, i.e. containing the evaluation of social functioning, the intervention plan (if applicable), the evolutionary notes and the intervention report;
• Enter relevant data into the appropriate information systems;
• Evaluate the needs for respite and emergency care, placement or housing and carry out the required interventions.
• Professional, interdisciplinary and partnership collaborations
• Register as a partner in the Local Service Network;
• Establish and maintain links with collaborators and partners;
• Promote consultation with internal and external collaborators and partners in order to develop the PSI or PII;
• Play the role of case manager or pivot intervenor in accordance with their specific mandate;
• Assume the responsibilities related to their collaboration in a multidisciplinary and interdisciplinary team;
• Ensure a role of consultant in their field of expertise to intervenors during interdisciplinary exchanges;
• Participate in the evaluation of the quality of services to the population and collaborate in its maintenance and improvement;
• Participate in the identification of means to update the program objectives;
• Collaborate in the orientation of new staff;
• May be called upon to develop and deliver training activities, according to their field of practice and with the agreement of their immediate superior.
• Perform any other function related to the request of their immediate superior.

Quality of practice and professional development

• Carry out their functions according to the quality standards and standards of practice in force;
• Optimize their practice through continuous training and mobilize their professional skills in order to provide quality services allowing the updating of the service offer.

Support for the updating of the university mission

• Collaborate in the supervision of interns, if they possess the qualifications required by the training institutions; (exception: auxiliary nurse and beneficiary assistant, because in these disciplines, the training institutions appoint their own internship supervisors);
• Facilitate the welcome and integration of all interns in their service, whatever their discipline;
• Contribute to the development of cutting-edge practices;
• Participate in research projects, with the agreement of their immediate superior.

SPECIFIC FUNCTIONS

• Evaluate the individual and/or couple social functioning of the client;
• Carry out short-term follow-ups for clients living in ambivalence regarding the continuation or not of a pregnancy;
• Support decision-making;
• Carry out short-term follow-ups for clienteles living difficulties of adaptation following an abortion;
• Evaluate the consent of clients living complex situations and throughout the process of voluntary interruption of their pregnancy;
• Evaluate and refer and/or accompany clients for the follow-up of other difficulties encountered;
• Participate in interdisciplinary meetings as needed;
• Intervene in punctual crisis situations;
• Carry out referrals to identified partners for advanced pregnancy abortions;
• Support, inform and carry out clients in their procedures to access an advanced pregnancy abortion;
• Support the entire IVG team in complex situations;
• Develop intervention and information tools to support clients in the process of voluntary interruption of their pregnancy;
• Collect information/statistics regarding referrals to identified partners;
• Collaborate in the advanced abortion coordination team, supports external partners in social evaluation and preparation of the file for referrals to appropriate resources.

Requirements

Must hold a bachelor's degree in social work or social service or meet the education requirements of the Order of Social Workers and Family Therapists of Quebec (OTSTCFQ).

Must be a member of OTSTCFQ.
