Website of the establishment: http://ciusss-estmtl.gouv.qc.ca Description Our long-term care facilities (CHSLD) have the mission of providing a substitute living environment, temporarily or permanently, to adult clients who can no longer remain in their natural living environment due to a loss of functional or psychosocial autonomy. This living environment provides residents with assistance, support, and monitoring services, as well as psychosocial, nursing, pharmaceutical, medical, and rehabilitation services. Essential aspects of the practice As a dietitian-nutritionist on the team, you will evaluate nutritional status, determine and implement an intervention strategy to adapt nutrition according to needs to maintain or restore health. You may also participate in clinical teaching.
Reserved activities: * Determine the nutritional treatment plan, including the appropriate feeding route, when an individual prescription indicates that nutrition is a determining factor in the treatment of the disease; * Monitor the nutritional status of individuals whose nutritional treatment plan has been determined. Professional competencies of clinical nutritionists: * Assess the presence of nutritional risk; * Assess nutritional status; * Determine a personalized nutritional treatment plan; * Communicate with the user and/or concerned parties regarding the nutritional treatment plan; * Monitor the nutritional status of the person whose nutritional treatment plan has been determined; * Record notes in the user's file; * Share expertise; * Contribute to research and professional development. Upon your arrival on the team, we will ensure you receive all the necessary support for the supervision of your functions and your professional development. Particularity of the position: * Multi-site replacement position serving the CHSLD Jean-Hubert-Biermans and Jeanne-Le Ber, with a part-time schedule equivalent to 0.9 day per two-week period.
