Social Work Technician - Mental Health Housing Follow-Up

Description

The housing follow-up team at the Montreal University Institute of Mental Health supports users with psychotic disorders living in mental health housing resources. Their mandate involves providing personalized psychosocial follow-up, coordinating necessary services, and collaborating with families and partners to ensure the well-being and stability of users. By working proactively, the team promotes their autonomy, quality of life, and recovery in a respectful and humane environment.

Upon arrival, personalized orientation modalities will help you familiarize yourself with the work, and mentorship will be provided to support your professional development.

More specifically, the main responsibilities of the social work technician within the team will be:

• Provide psychosocial follow-up for users housed in mental health resources.
• Assess users' needs to make requests for housing or relocation, as appropriate.
• Develop individualized intervention plans (P.I.I.) and coordinate services based on established goals.
• Organize and facilitate follow-up meetings with families to promote active collaboration.
• Work in collaboration with internal and external partners to provide integrated and tailored services.
• Contribute to maintaining users' stability and quality of life through a human-centered approach focused on their needs.

Qualifications

• Hold a college diploma (DEC) in social work techniques;
• Hold a valid driver's license.
