Job Offer: Psychosocial Worker – Short-Term Service

Full-time permanent position – 37.5 hours per week

At Diogène Suivi Communautaire, we believe that every person has strengths, abilities, and potential for development.

For over 35 years, we have been supporting people living with severe mental health issues, often in situations or at risk of homelessness, to regain a sense of agency, access stable housing, and build the life they desire.

To meet the growing needs of the community, we are expanding our team and are looking for a psychosocial worker for our Short-Term Service.

About the Short-Term Service

The Short-Term Service offers intensive, rapid, and targeted support to people experiencing a crisis or significant vulnerability in mental health.

The goal is to quickly stabilize the situation, prevent a disruption in the life journey (loss of housing, hospitalization, isolation, disorganization, etc.), and promote a resumption of agency.

The people we support often have mental health issues, substance use concerns, housing precarity, or social isolation.

Interventions are offered directly in the living environment and are adapted to the intensity of the needs, in an approach of proximity, recovery, and harm reduction.

Your Role

As a psychosocial worker, you will be called upon to:

• Conduct psychosocial and functional assessments of the situation;
• Establish a rapid trust relationship with the people being supported;
• Develop a short-term action plan centered on the person's priorities;
• Provide intensive support aimed at stabilizing the situation;
• Mobilize the resources of the health network and community partners;
• Plan transitions to appropriate services when necessary;
• Participate in clinical meetings;
• Write progress notes and clinical documentation;
• Contribute to the development of the organization's practices.

Requirements

• DEC or bachelor's degree in a relevant field (social work, psychoeducation, special education, criminology, psychology, or any other relevant training);
• Experience in mental health, crisis intervention, homelessness, substance use, or community intervention (a significant asset);
• Good knowledge of recovery, harm reduction, and Housing First approaches (an asset);
• Valid driver's license (an asset);
• Good knowledge of the health network and community resources;
• Excellent mastery of spoken and written French.

Compensation and Benefits

• Salary starting at : $28.14/hour to $37.60
