Description

The CIUSSS de l’Ouest-de-l’Île-de-Montréal is looking for a nurse to fill a full-time day shift position and join the intensive follow-up team in the community for the entire territory of the Ouest-de-l'Île.

The intensive follow-up in the community is a community psychiatry service focused on treatment and recovery. Multidisciplinary services are provided by a team of professionals including social workers, criminologists, nurses, specialized educators, and psychiatrists.

Follow-up is offered in the community for adult individuals with severe and persistent mental health disorders, which may also include other biopsychosocial health problems (e.g., physical health issues, substance use, residential instability, contact with the justice system, etc.). These individuals are often severely functionally impaired and their instability makes them vulnerable to repeated hospitalizations or social isolation (e.g., homelessness, legal issues), hence the need for interventions several times a week.

Working Hours: Monday to Friday from 9:00 to 17:00, 12:00 to 20:00.

One (1) Sunday out of four (4) from 12:00 to 20:00.

Tasks and Responsibilities

  • Assess the user's health status, determine and implement the intervention and nursing treatment plan;
  • Primarily intervene in the community with the user, using an interaction-centered and social functioning restoration approach, with the goal of social reintegration, empowerment, and recovery;
  • Manage the medication system, administer and document medical treatment;
  • Update the nursing treatment plan as needed;
  • Coordinate services with other partners providing medical care (emergency, healthcare facilities, dentists, etc.);
  • Participate in activities related to teaching, prevention, and health promotion (teaching on diabetes, teaching on healthy lifestyle habits, discussion on risky behaviors);
  • Contribute to team training on recognizing psychiatric symptoms and medication side effects (cross-training);
  • As part of our university and academic mission, you may be asked to supervise students or interns.

Qualifications

  • Hold a college diploma in nursing or an equivalent diploma recognized by the competent authorities;
  • Be a member of the Ordre des infirmières et infirmiers du Québec (OIIQ);
  • Have relevant experience with an adult clientele presenting severe and persistent mental health disorders such as schizophrenia, bipolar disorder, major depression, schizoaffective disorder, and concomitant disorders;
  • Be able to make trips in the clientele's living environment;
  • Be able to establish a therapeutic relationship with a bilingual clientele.
