The CIUSSS of Centre-Ouest-de-l'Île-de-Montréal is looking for a clinical nurse to join the team at CLSC Métro within the Direction of the program for support to the autonomy of the elderly (SAPA).
In this role, you will contribute to maintaining users at home, assessing their needs, and coordinating quality care, while collaborating closely with an interdisciplinary team.
What you can expect
- ✅ Assess the health status and needs of users
- ✅ Provide clinical follow-up for clients at home and in RI-RTF
- ✅ Develop and adjust care and intervention plans
- ✅ Collaborate with various professionals in the network and community partners
✅ Participate in prevention and intervention activities related to the maltreatment of the elderly (LAMAA) ✅ Contribute to the continuous improvement of the quality and safety of care
