Social Worker - Guimont (LAVAL) - Permanent - Full-time

Summary of Duties

• Under the responsibility of the program administrator, the incumbent:
• Provides psychosocial support to the user and their family in order to meet their needs and promote acceptance and understanding of the user's diagnosis;
• Collaborates with the interdisciplinary team in the development, evaluation, and implementation of programs and intervention plans;
• Involves and sensitizes the user's family members and the surrounding community in the implementation of the intervention plan;
• Directs the user and their family towards the various resources that can meet the user's needs;
• and applies the regulatory and legislative provisions related to their field of activity;
• Advises and informs various intervenors and the client base on regulatory and legislative provisions;
• Initiates, supports, and implements development or research projects that meet the needs of the client base;
• Participates in the orientation and training of new staff and interns;
• Performs any other tasks related to the request of their immediate supervisor.

SPECIFIC FUNCTIONS TO THE SERVICE

• The incumbent completes the social functioning evaluations of users and families presenting problems in the accompaniment of people with intellectual disabilities and serious behavioral disorders in order to identify the problems, the way to solve them and find solutions, she:
• Uses her therapeutic skills to involve users and families in therapeutic activities in order to reduce the impact of the disability, promote independence and improve well-being;
• Provides support and advice, mobilizes users as well as their families who feel behavioral and/or emotional vulnerabilities and helps them to adapt to their situation;
• Deals with the sensitivity of families and users who demonstrate high levels of distress, anxiety, confusion and aggression by treating them with dignity and respect;
• Participates in continuous evaluations and adjustments of clinical interventions based on the changing needs of users;
• Provides advice and support at the times of transitions that occur during the different stages of the user's life cycle as well as at other times of transition of the users and their families based on the needs of the users (ex.: change of placement);
• Provides her support as well as the opportunity for families to make choices, changes and understand the treatment options in relation to the individual service plans (ISP) and intervention plans (IP) of the user;
• Provides advice and support to help families understand the diagnosis as well as the prognosis which have an impact on the planning of the user's services;
• Provides support to staff and families presenting difficulties in dealing with death, an incurable disease, etc…;
• Based on the reference system, provides support and advice to help resource managers understand the implementation of the clinical tools inscribed in the intervention plans and the individual service plans of the users. This applies particularly to users under public guardianship or without family;
• Provides practical help to users and their families in relation to the application of certain legislations (ex.: public guardianship, administrative application forms for subsidies, community resources, etc.); she also acts as a liaison agent with other health and social services networks and community organizations;
• Ensures that the rights and self-determination of users are respected;
• Shares a teaching responsibility with users, families and staff;
• Ensures supervision during the placement of interns in social work;
• Sits on different committees within the organization (ex.: multidisciplinary council);
• May participate in research projects related to the psychosocial aspects of users and families of the organization.
